Individual Package

  • $95 per month

  • 4 visits for 1 individual

 

Family Package

  • $170 per month

  • 4 visits each for 2 family members

Family Plus Package

  • $220 per month

  • 4 visits each for 3 family members

OR

  • $265 per month

  • 4 visits each for 4 family members

Urgent Injury Appointment

(Provides additional time to treat an injured patient.)

  • $20 surcharge (and one package visit)

Established Patient Adjustment

  • $50 for single visit

 

New Patient Adjustment

  • $75 for single visit

New Patient Individual Package (1st Month Only)

  • $135

  • 4 visits for 1 individual (includes consultation)

After Hours Emergency Appointment

(Appointment during non-business hours; must be requested via email and confirmed upon availability. Fee separate from package pricing.)
